TELEHEALTH PATIENT INFORMATION
Telehealth is an exciting new service that we are offering for people that are unable to attend a Physiotherapy appointment in person due to the current COVID-19 situation. This gives you access to one of our Physiotherapists from the comfort of your own home.
If you are a new patient considering a Telehealth appointment, please contact our office on (02) 4385 6166 to discuss whether this is an appropriate option for you and to make an appointment.
Our reception staff will then email you a new patient information form that we would appreciate if you could please print and fill out. Please make sure you have included a phone number that we can reach you on should we need to.
Once completed please scan the form and resend back to email@example.com
If you are a new patient, prior to your appointment you will receive an email from us inviting you to either download the free app called “Physiapp” for your phone. If you would rather undertake the consultation on your computer you can use www.physiapp.com and log in with the details in the email.
If you have previously seen a Physio in our clinic please log into Physiapp using the log in that was originally sent to you when your Physio first set you up on “Physiapp”.
Please note that whichever device you choose to use, it is required that the device has a camera so the physiotherapist can see you.
On the day of your appointment
- Please wear appropriate clothes to move around in - The Physio may ask you to perform certain movements to assess your injury, plus the Physio will ask you to perform certain exercises that they will prescribe as part of your home exercise program.
- Please choose a room that is QUIET and has enough space to move around in.
- Please choose a room with adequate light - Please orientate yourself in the room so the main source of light is NOT coming from behind you.
Please log into “Physiapp” 10min prior to your appointment time, to allow for time for you to call our receptionist should you be having difficulty with the log in process.
Prior to your appointment you will be emailed an invoice, we require that the invoice is paid for in advance of the Telehealth appointment. You can either call our office and pay over the phone, or you can pay via our website by hovering on the “Telehealth” menu and clicking the “Telehealth online payment” sub-menu item and filling out the payment form. We will then send you a receipt.
Please note that if you are in a Private Health Fund, some Funds have approved certain Telehealth appointments to be eligible for a rebate. You will need to contact you Health Fund Personally to see if you are eligible. Unfortunately our staff are unable to tell if you are eligible or not.
